Form A Filing Instructions For District-Level Delegates
Filing Period
Form A (District-Level Delegate) filing period is from January 15 through April 2, 2008 at 5pm.
Filing Method
You may file online or you may print a blank Form A, fill it out manually and fax to (916) 442-5715.

Before Filing
Prior to filing Form A you should know your Congressional District or name of your Congressmember, your Senate District or name of your State Senator and your Assembly District or name of your State Assemblymember. Click here to find this information using your zip code + 4 or your address.
After Filing
Upon filing Form A online you will be prompted to print a copy. You will need to have the latest version of Adobe Reader to download and view this file. (Note to Macintosh users: the built-in Preview application cannot view this file. You will need to have the latest version of Adobe Reader installed and setup as the default PDF viewer to access this file.) Please install Adobe Reader before beginning Form A.
After printing Form A, sign it and fax to (916) 442-5715. Or you can send by mail to:

Please keep a copy for your records.
You will receive a confirmation email that will confirm that your Form A has been received online. It will contain the date and time your Form A was filed online and the candidate you have pledged to support. Please keep a copy of this email for your records.
You may receive a second email from the CDP to verify your email address. Clink on the link in that email to confirm your email address so you can receive information from the CDP.
Date & Time Filed
An applicant’s Form A is considered “filed” as of the date and time the form is filed online or received (via fax) by the CDP.
Filing of Form A Officially Complete
Form A filing is not considered officially “complete” until the signed Form A is received by the CDP by either fax or mail. If sent by mail, it must be postmarked by April 2, 2008.
List of District-Level Delegate Candidates
A list of District-Level Delegate candidates will be posted on the CDP website, listed by candidate pledged within each congressional district.
Additional Information
Candidates running as district-level delegates will be notified by email of additional information they may need to know, such as the April 13 caucus sites and voting information. Please add delegate08@cadem.org to your "approved sender" email list.
